It's sad (and ironic) that this department is CLOSED during the COVID-19 pandemic. No email is available and the number provided is useless as no one is in the office. I am trying to sort out my child's immunization records and without a OHIP number or immunization number it is impossible to sign in to the online tool. I don't think this is acceptable from a government department given it should be available to the public, especially during a health crisis.

The location is no more TPH immunization information center , it’s a empty building.Had better to mention it’s closed ,Otherwise we went a way and noticed that .

Rediculis . I've sent in copies for 3 straight years and they keep asking for copies of my almost 15 year old daughters shots...Let's say she dident get them ( which of course she did)They would be almost 4 years late protecting the other children
