Breast Screening Belleville & Trenton Centre

Trying to set up a mammogram, but I need there specific form. I have a form from my doctor but that isnt good enough. Dr’s don’t have time to fill different forms for different institutions. Then she wanted the form, so I am suppose to drive 30 minutes to drop of a form?? Really.And of course I can’t scan it and send it to her because they wouldn’t accept that so I think that these institutions need to come in to 2023 and get with the program. I could fax it, but honestly who faxes anymore.This is a very frustrating process.
